Fact SheetsWhat is it? The Fact Sheet contains introductory information about Q Fever and the Australian Q Fever Register, targeted at employees involved in a screening and vaccination program. How to use it? A copy of the Fact Sheet should be given to each employee in a screening and vaccination program, along with a copy of the Personal Details and Consent Form FormsWhat is it? This form must completed by individuals before their information is put on the Register. It contains their personal details (used to uniquely identify them), explains the use of the Register, and indicates that they have given their consent for their Q fever status information to be put on the Register. Note:The form consists of two pages, which may be printed front and back of a single sheet, or on two pages stapled together. Do not distribute the first page of this form without the second. How to use it? Distribute a copy of this form, along with a copy of the Q Fever and the Australian Q Fever Register - Fact Sheet to all employees or clients participating in a screening and vaccination program. What is it? Organisations wishing to submit data to, or query data from the Register should complete one of these forms. It contains the details of the organisation and describes the requirements of access to the Register. How to use it? The form should be completed and signed by an authorised representative of the organisation, after reading the Terms and Conditions attached to the form. In addition to the Application for Access one or more User Registration Forms should be completed. All forms should then be mailed to the Register. What is it? This form contains the details of individuals nominated by their organisation as users of the Register. How to use it? Complete one form for each nominated user, and send it to the Register, along with the Application for Access form. What is it? This form contains the key information required about a person during a screening and vaccination program. It is designed for use by Occupational Health and Safety staff. The same form may be used to record information required by the Register, and State Departments of Health. How to use it? Download the instructions before use. Use one form for each person in the screening and vaccination program. When screening and/or vaccination is complete, the form can be used to enter the relevant data onto the Register. Record the Register File Reference numbers in the appropriate place on the form, and mail a copy to the Register, along with the Personal Details and Consent Form What is it? This document provides instructions for use of the previous form. How to use it? Read this before using the Pre-screening and Vaccination Form. What is it? This questionnaire is used at the start of a pre-screening and vaccination program to determine if a person has been previously tested, vaccinated, diagnosed with Q fever, or has had high risk previous exposure. How to use it? This form is not required by the Australian Q Fever Register. It may be used by organisations when running a screening program to determine whether an individual is eligible for inclusion in the program. What is it? The Information Release Authority is used to indicate that a person has given an organisation authority to seek information on previous testing, vaccination or disease diagnosis from another organisation. How to use it? This for is not required by the Australian Q Fever Register. It may be used by an organisation if a person indicates that they have been diagnosed with Q fever or have participated in a screening and vaccination program before. The person should complete and sign this form, after which it may be sent to the organisation that conducted the program, seeking confirmation of the person's immune status. This is a Microsoft Excel 97 spreadsheet with standard data entry templates for batch data submission and queries. This allows all the data entry for the Register to be done while you are not connected to the Internet. Once connected, all the data can be submitted in one quick step. How to use it? Save the spreadsheet to your hard disk and enter the data into the appropriate page. When finished, log on to the Register Web site, and select Submit batch data or Batch query from the main menu. Highlight and copy the data from your spreadsheet, and paste it into the Web page. System DocumentationWhat is it? The manual contains full documentation of the Australian Q Fever Register Web site. This is an electronic copy of the Manual distributed to all participating organisations. How to use it? Use the flow charts for common operations, the site map to understand the structure of the Web site, or read the step by step instructions for each type of task. What is it?
